Chronic Care Registered Nurse
Job Description
Salary: $70-75K

What your future day will look like:
- Manage quality of care for chronic care patients
- Close the gaps between the providers and patients
- Follow up on medications, screenings, and chronic care management goals
- Train other clinical staff as needed
- Establish & implement treatment plans consistent with standard of care.
- Weekly meetings

Type: Direct Hire
To be a champion in this role, you will need:
- 5+ years of RN experience with some prior experience to include chronic care in a physician office setting
- Current SC RN license
- Excellent written and verbal communication skills
- Work Monday-Friday 8a-5p
